Privacy Notice
I am registered with the ICO.
This document sets out your rights and how I comply with Data Protection Law concerning information I hold about you. The purpose of data collection and retention relates to the professional service I provide to you.
Information I collect and hold
1. I collect information which includes your name, postal address and telephone numbers, email address, details of your GP, psychological medication prescribed, and details of any other relevant health professionals engaged in your care. Further information will include the days and times we meet, duration of the work, personal and family history, and information regarding relationships, matters regarding risk and safeguarding issues.
2. I will obtain a signed medical consent form and contract when the work begins.
3. I would only contact your GP/medical practitioner or others with your consent, unless it was considered you or others to be at risk.
4. I will retain a record of your consultation for administrative purposes and should you re-present for further assistance at a future date.
5. Brief anonymised notes will be kept on your sessions in order to assist the process of clinical supervision. They will be safely destroyed as soon as they are not useful.
6. A record of attendance and payment will be held.
7. Records will be kept for a maximum period of 10 years.
Protecting information about you
1. I make every effort to treat and store securely all information about you – on paper and held electronically.
2. In order to ensure good professional practice, I will occasionally consult with clinical colleagues about your treatment, in which case all personal identifying information is anonymised to protect your identity whenever necessary.
3. In the rare event that I am unable to continue your treatment, an SAP colleague (clinical trustee) would be provided with your contact details in order to speak with you.
4. Your information is only seen by me, or a clinical trustee. All clinicians are subject to the SAP and BPC codes which require compliance with ethical standards, including respect for confidentiality.
Why I collect and hold your information
1. I only collect information relevant to the therapeutic work with myself.
2. All information held relates to my work and may be used for training, statistical and research purposes. If this is the case, all identifying information will be anonymised.
3. Application, consultation and treatment information and correspondence is retained in case you re-present for further assistance at a later time.
Your rights
1. You have the right to see any information we hold about you.
2. You have the right to request that information we hold about you is deleted once your treatment has been completed.
3. You also have the right to require that information I hold about you is rectified if it is inaccurate or incomplete.
4. To access information and to request rectification or deletion, you may raise this with myself either in person or by email amallen5@btinternet.com
Data management and complaints
Should you be dissatisfied with the way Anne Marie Allen has managed your information and your complaint, you may submit a complaint to the Office of the Information Commissioner.
